Summary

A Cochrane systematic review and meta-analysis comparing automated closed-loop weaning systems with non-automated weaning strategies in critically ill adults and children. Twenty-one trials including 1,676 participants were pooled, with outcomes of weaning duration, total ventilation duration, ICU and hospital length of stay, mortality and adverse events. Automated systems were associated with reduced weaning and ventilation duration and shorter ICU stay, though heterogeneity between trials was substantial. The authors call for an adequately powered, high-quality multicentre randomised trial; paediatric data are limited within the pooled population.
