Summary
A systematic review appraising 18 clinical practice guidelines and 170 recommendations on the management of pain, sedation, delirium and iatrogenic withdrawal syndrome in the PICU, published 2010–2023. Guideline quality was assessed with AGREE II and AGREE-REX, recommendations were synthesised thematically into 30 summary statements, and strength of recommendation and certainty of evidence were harmonised using GRADE methods. Most guidelines were of medium quality, with assessment-related recommendations showing the greatest consistency while others were conditional, inconsistent or poorly supported by evidence. Highlights gaps in the evidence base, patient and family involvement, and implementation resources for PICU sedation and comfort practice.
