Summary

A secondary analysis of over 10,000 patient-days from the multicentre PICU Up! pilot randomised trial examining the dose-response relationship between sedative exposure and next-day delirium in critically ill children. Higher previous-day benzodiazepine dose was associated with increased odds of delirium, with the steepest rise at the lowest doses and no safe threshold identified. Any dexmedetomidine exposure and same-day physical restraint use were also associated with increased delirium odds, while opioid dose was not. Supports minimising benzodiazepine use and individualised sedation and restraint practices in PICU.
