Summary
ESPNIC clinical practice guidelines on intravenous maintenance fluid therapy for acute and critically ill children from 37 weeks gestation to 18 years, based on a systematic review and meta-analysis with SIGN grading. Five questions were addressed: indications for IV maintenance fluids, use of isotonic fluids, balanced solutions, fluid composition (including calcium, magnesium, potassium, glucose and micronutrients) and optimal volume. Sixteen recommendations were produced with high expert consensus despite heterogeneous evidence quality. Key themes include using isotonic balanced glucose-containing solutions, restricting infusion volumes in most hospitalised children, and regular monitoring of electrolytes, glucose and fluid balance.
