Summary

Systematic review and meta-analysis comparing balanced versus unbalanced (0.9% saline) crystalloid bolus therapy in critically ill children. Thirteen studies were included, with three randomised trials (162 patients) pooled for the primary outcome of change in serum bicarbonate and pH within 24 hours. Balanced fluids were associated with modestly greater improvement in bicarbonate and pH after 4-12 hours, with no differences seen in chloride, acute kidney injury, renal replacement therapy or mortality. The authors conclude that adequately powered trials are needed before clinical recommendations can be made.
