Summary
A systematic review and meta-analysis of 17 observational studies (2,725 infants) describing mortality risk factors and organ support requirements in critically ill infants with pertussis admitted to PICU, with validation in a single-centre cohort of 83 patients. Pooled estimates are reported for mechanical ventilation, continuous renal replacement therapy, ECMO use and mortality. Risk factors associated with death included elevated heart rate, pulmonary hypertension, seizures and raised white cell count, which were combined into a simple mortality prediction score. Relevant to PICU teams stratifying severity and considering escalation, including extracorporeal support, in malignant pertussis.
