Detection of intracranial hypertension in children using optical coherence tomography: a systematic review.

- Raised intracranial pressure
Summary
A systematic review of 21 observational studies assessing whether optical coherence tomography (OCT) can detect intracranial hypertension in children under 18, including those with craniosynostosis, idiopathic intracranial hypertension and space-occupying lesions. OCT measures of optic nerve, rim (including retinal nerve fibre layer) and retinal parameters showed reasonable diagnostic capability alongside established measures of intracranial pressure. Study quality was fair to poor with no level 1 evidence, and parameters and reference standards varied widely. The authors call for prospective validation before formal clinical guidance can be developed.
