Summary

A prospective cohort study at a tertiary paediatric medical-surgical ICU measuring adherence to 19 evidence-based clinical care guidelines across 814 patients and over 4500 compliance events. Overall compliance was around 78% but varied widely, with high adherence for stress ulcer prophylaxis and blood product administration and low adherence for ventilator-associated pneumonia prevention and vitamin K administration. Guidelines with simple binary decision points or single-page flow diagrams were followed more often, while poor compliance related to perceived untrustworthiness of guidance and time pressures. Useful for PICU teams interested in quality improvement and guideline implementation.
