Summary

A single-blind randomised trial in 50 Indonesian children with dengue shock syndrome comparing WHO-recommended Ringer's lactate resuscitation with a smaller-volume hypertonic sodium-lactate solution. Haemodynamic recovery, plasma expansion, clinical outcome and survival were similar between groups, while fluid accumulation was lower with hypertonic sodium lactate and endothelial marker sVCAM-1 fell. Relevant to PICU teams managing fluid-refractory shock and concerns about fluid overload, particularly in dengue-endemic settings.
