Summary

Systematic review and meta-analysis of four randomised controlled trials (454 patients) comparing restrictive versus liberal red blood cell transfusion thresholds after paediatric cardiac surgery. Outcomes assessed included ICU and hospital length of stay, duration of ventilation, arterial lactate and mortality, with no significant differences found between strategies. The authors conclude restrictive thresholds appear non-inferior, but the evidence base is limited and further high-quality trials are needed. Relevant to PICU teams managing postoperative congenital cardiac patients and transfusion decision-making.
