Summary
Secondary analysis of the PENUT randomised trial examining fluid balance patterns in 874 extremely premature neonates born at 24-27 weeks across 30 US neonatal units. Peak fluid balance during the first 14 postnatal days, expressed as percentage weight change from birth weight, was related to mechanical ventilation on day 14 and to the composite of severe bronchopulmonary dysplasia or death. Higher peak fluid balance was seen in those still ventilated at day 14, and the authors suggest early fluid balance and time to return to birth weight as potential management targets. Relevant to neonatal and PICU clinicians managing fluid strategies in ventilated infants.
