Summary
A systematic review, meta-analysis and trial sequential analysis of vasopressin and terlipressin as rescue therapy in refractory shock in neonates and children, compared with conventional fluid and vasoactive therapy. Eight studies including 224 patients were pooled, with mortality, PICU length of stay and tissue ischaemia as main outcomes. No benefit for mortality or length of stay was observed, and the authors raise concern about a possible increased risk of ischaemic events. Trial sequential analysis indicates the evidence base is underpowered and larger trials are needed.
