Summary

Systematic review and meta-analysis of eight randomised trials (653 children) comparing high-flow nasal cannula oxygen with conventional oxygen therapy across the perioperative period, covering anaesthetic induction, intraoperative sedation and post-extubation. HFNC was associated with fewer desaturation events, higher minimum oxygen saturation and less need for additional airway interventions, with no significant difference in safe apnoea time or end transcutaneous CO2. Findings are limited by few studies and heterogeneous clinical settings. Relevant to PICU teams using apnoeic oxygenation and HFNC around intubation, procedural sedation and extubation.
