Summary

Subanalysis of the PEPaNIC randomised trial focusing on the 20% of critically ill children identified as undernourished on PICU admission. It compared supplemental parenteral nutrition started within 24 hours with withholding PN for one week when enteral intake was insufficient. In this subgroup, late PN was associated with fewer new PICU-acquired infections and shorter PICU stay, with similar mortality, hypoglycaemia and weight deterioration. Relevant to PICU nutrition practice, suggesting undernourishment on admission does not mandate early parenteral supplementation.
