Summary

A multicentre Dutch observational study examining how a 2020 national guideline for high-flow nasal cannula therapy in bronchiolitis was implemented across hospitals referring to one PICU. Local protocols were compared with the national guideline and paediatricians were surveyed about their use of HFNC. Considerable variation was found in initiation criteria, weaning practice and use of pCO2 for assessment, with practice deviating from both national and local protocols. The authors conclude guideline implementation was ineffective and clinical practice remains non-uniform.
