Summary
Systematic review and meta-analysis of tracheostomy practice in mechanically ventilated children, covering timing, indications, complications, mortality and decannulation success. Sixty-six studies were reviewed qualitatively with eight contributing to a meta-analysis of early versus later tracheostomy. Early tracheostomy (within the first 14 days of ventilation) was associated with fewer ventilator days and shorter hospital stay, but no difference in in-hospital mortality. The authors suggest timing decisions may depend more on the underlying disease than on duration of ventilation.
