Summary
Secondary analysis of the PEPaNIC randomised trial examining whether fasting-induced ketogenesis mediates the benefits of withholding early parenteral nutrition in critically ill children. Plasma 3-hydroxybutyrate and regulators of ketogenesis (glucose, insulin, glucagon) were measured in matched and total cohorts, with mediation analyses adjusted for randomisation and baseline risk. The authors report that late parenteral nutrition raised plasma 3HB, which statistically mediated part of the outcome benefit (fewer infections, earlier weaning, shorter PICU stay). Relevant to PICU nutrition practice and understanding of macronutrient timing.
