Summary

A modified three-round Delphi among 36 international experts developed PROACTIVE, a tool to assess the capacity and quality of paediatric onco-critical care, particularly in resource-limited settings. It comprises 119 indicators across eight domains and 22 subdomains covering structure, performance and outcomes. Highest-ranked indicators included a designated PICU area, availability of a paediatric intensivist, involvement of haematology-oncology physicians in the primary team, critical care-trained nursing staff, and timely PICU transfer. Intended for service evaluation and quality improvement rather than bedside clinical management; validation against outcomes is planned.
